As a courtesy to facilitate prompt treatment, our office will file claim forms for patients and assist patients in every way we can. After we verify that the patient’s insurance covers acupuncture, we will bill your insurance company and charge you only the required deductible, co-pay or co-insurance. If Dr. Lok is not contracted with the patient’s insurance company, and it covers out-of-network acupuncture and related services, you will be charged our regular rates and our office will provide a claim form for submission to your insurance.
Call our office at (702) 732-0178 if you are unsure about your coverage. Our staff will be happy to verify your insurance and discuss your benefits or payments.
Patients can also call the phone number on the back of their insurance card, or simply call our office at (702) 732-0178 and ask our staff to verify the insurance.
