New Patient Intake Form – Required

This form lets us know the history and current state of your health so that we can better serve you. Please follow the steps below before your first appointment.

New Patient Intake Form

1. New patient who wants to schedule and/or fill out INTAKE FORM has to “SIGN UP” to create an account.
2. Active patients who wants a PATIENT PORTAL ACCOUNT for scheduling or updating INTAKE FORM has to go to ‘SIGN UP” to create an account.
3. Active patients with a PATIENT PORTAL ACCOUNT who wants to schedule or update INTAKE FORM has to “LOG IN WITH ATHENAHEALTH”.
